It is the third film in the Dollars trilogy following A Fistful of Dollars (1964) and For a Few Dollars More (1965). The plot centers around three gunslingers competing to find a fortune in buried Confederate gold amid the violent chaos of gunfights, hangings, Civil War battles, and prison camps.

The film stars Sean Patrick Flanery and Norman Reedus as Catholic Irish American fraternal twins, Connor and Murphy MacManus, who become vigilantes after killing two members of the Russian Mafia in self-defense. After a message from God, the brothers, together with their friend David Della Rocco, set out to rid their home city of Boston, Massachusetts of crime and evil, all the while being pursued by FBI agent Paul Smecker (Willem Dafoe). It was divisive among viewers, ultimately developing both a large cult following.
